5/28/21 ID# 73 - Critical Care Medflight Registration Form was submitted, with the incorrect information. Bambi asked Amy to delete this form because she created a second submittal for the registration. I did delete this and should have expired the document. The registration form is printed in an email format and saved in MHP's Registration folder for record retention.
1/1/22 - Registration Fee has changed to $1000
